申請入學最多可申請三個系所，但僅能選擇一系所辦理報到及註冊入學。申請兩個系所以上者需分別繳交申請文件，但可合併寄送。

Each applicant can apply no more than three departments or institutes at the same time. Those who apply for two or three departments need to provide the application documents separately, but all the documents can be submitted together in a single packet. If accepted by all the departments applied, however, the student may choose only one program to enroll in.

1. 通訊報名

將申請資料郵寄至：台灣台中市40201建國北路一段110號 中山醫學大學教務處，並於信封黏貼申請入學「報名專用信封封面」並詳填相關資料。

2.  電子郵件

在2025年3月31前將報名資料完整PDF檔(四、申請文件P4~5)以信件主旨《114學年度外國學生申請入學-考生姓名》傳送，資料檔案總容量最多以 20MB 為限。

3. 現場報名

於申請截止日前親自送申請資料至「中山醫學大學教務處」。

※ Ways to Apply

1.By mail: Submit all your required items of things by mail to the Office of Academic Affairs, Chung Shan Medical University (Address: No. 110, Section 1, Jianguo N. Rd., Taichung City 402, Taiwan, R.O.C.) before the application deadline.  Please attach the application cover sheet format to the outside of the envelope.

2.By E-mail: Convert the application documents into a single PDF file in sequence listed on pages 4~5 via email with the subject line "2025–2026 International Student Admission Application - [Applicant's Name]" before March 31, 2025. The total size of the attached files must not exceed 20MB.

3.In person: Submit all your required items of things in person to the Office of Academic Affairs, Chung Shan Medical University before the application deadline.

In person:

***.申請人須具備及提供之語文能力證明文件(依申請系所學制之規定) (影本)。

*** Applicants must possess and submit language proficiency certificates in accordance with the regulations of the applied program or department. (photocopy)

***.請銀行出具足夠在臺就學之財力證明(最低額度為美金一萬元)，或由政府、大專校院或民間機構提供全額獎助學金之證明

        Financial proof by Bank that shows financial sustainability for study in Taiwan (the minimum balance should be equivalent to US$10,000), or proof of full scholarship provided by a government agency, university, college, or private organization.

※ 若存款證明非申請人本人帳戶，另須檢附資助者親筆聲名之財力保證書(附件三)，說明資助者與申請人之關係，並保證負擔申請人在臺就學所有費用。

If the certificate is not in the name of the applicant, a signed sponsorship letter (in Chinese or English) is required (Appendix 3), in which the relationship between the sponsor and the applicant is stated and the support of the applicant’s expense for the duration of the applicant’s studies in Taiwan is guaranteed.
