2026年1月10日至3月31日(January 10 to March 31, 2026)

申請入學最多可申請三個系所，但僅能選擇一系所辦理報到及註冊入學。申請兩個系所以上者需分別繳交申請文件，但可合併寄送。

Each applicant can apply no more than three departments or institutes at the same time. Those who apply for two or three departments need to provide the application documents separately, but all the documents can be submitted together in a single packet. If accepted by all the departments applied, however, the student may choose only one program to enroll in.

入網路報名系統（http：//aca.csmu.edu.tw）→點選報名→選擇報考學制→輸入考生「統一證號或護照號碼」及「自訂8位數密碼」→進入「網路報名填寫」→輸入報名資料及上傳相片並存檔

Access the online registration system (http://aca.csmu.edu.tw).

→ Click on "Registration."

→ Choose the desired academic program.

→ Enter the applicant's "Passport Number" and create an "8-digit custom password."

→ Proceed to "Online Registration Form."

→ Enter registration details and upload a photo, then save the information.

2.申請文件繳交 Application Document Submission

(1) 通訊郵寄 By mail:

將申請資料郵寄至：台灣台中市40201建國北路一段110號 中山醫學大學教務處，並於信封黏貼申請入學「報名專用信封封面」並詳填相關資料。

Submit all your required items of things by mail to the Office of Academic Affairs, Chung Shan Medical University (Address: No. 110, Section 1, Jianguo N. Rd., Taichung City 402, Taiwan, R.O.C.) before the application deadline.  Please attach the application cover sheet format to the outside of the envelope.

(2)電子郵件 By E-mail

在2026年3月31前將報名資料完整PDF檔(四、申請文件P5~7)以信件主旨《115學年度外國學生申請入學-考生姓名》傳送，資料檔案總容量最多以 20MB 為限。

Convert the application documents into a single PDF file in sequence listed on pages 5~7 via email with the subject line "2026–2027 International Student Admission Application - [Applicant's Name]" before March 31, 2026. The total size of the attached files must not exceed 20MB.

3.現場報名 In person:

於申請截止日前親自送申請資料至「中山醫學大學教務處」。

Submit all your required items of things in person to the Office of Academic Affairs, Chung Shan Medical University before the application deadline.

申請學歷說明：Educational (or academic) background

1. 符合教育部採認之高中（非本國高中）或在我國境內就讀外國僑民學校或我國高級中等學校附設之雙語部（班）或私立高級中等以下學校外國課程部班畢業者、大學或獨立學院畢業者。申請大學部者需具高中畢業（含應屆畢業生）學歷；申請碩士班者需具學士學位；申請博士班者需具碩士學位。

To apply for entering a bachelor’s-degree program, the applicant must have a graduation diploma from an overseas high school or from a foreign high school or from a foreign curriculum department and classes at private senior high schools in Taiwan which has been recognized by our Ministry of Education.  To enter a master’s-degree program, the applicant must be a holder of a bachelor’s degree.  To enter a doctor’s-degree program, the applicant must be a holder of a master’s degree.

2. 具有與我國學制相當之同等學力資格者。

Applicants with equivalent qualifications to the academic degree system of Taiwan can apply for admission.

※入學前畢業年級相當於國內高級中學等學校二年級之國外或香港澳門地區同類學校畢業生，其畢業學分應增修二十學分。

  Those having completed study at an equivalent foreign or Hong Kong/Macao high school which is equal to the sophomore of a senior high school in Taiwan may apply to the undergraduate program at CSMU, however, according to the Regulations of CSMU, their required number of credits for graduation shall be increased by 20 credits to complete their study.

申請文件 Items of Things to be Submitted

1. 繳交資料檢查表（附件一）一份。

   Application checklist (Appendix 1)

2. 本校入學申請表（附件二）一份。

   One copy of the application form (Appendix 2).

3.申請費用：免費。

   Application fee: Free.

4. 護照影印本一份及出生證明影印本。（日、韓籍人士可檢附全戶戶籍謄本代替出生證明。）

（依據教育部95.03.09台文字第0950033053號來文，轉知內政部之查詢國籍身分認定程序；請各位申請人務必繳交您的身分證明、生父母身分證明、國籍證明、出生及親子關係證明等相關文件。）

 A photocopy of the applicant’s passport and a copy of the applicant’s birth certificate.  (Japanese and Korean applicants can submit household registration transcripts instead of birth certificates.)

*According to regulations of the Ministry of Education and the Ministry of the Interior, applicants must submit his/her birth certificate, photocopy of parents’ identity card and any document that proves their parental relationship. Documents that certify the applicant’s nationality are also mandatory.

1. 學歷證明文件及成績單：

Academic credentials and academic transcript

經我國駐外使領館、代表處、辦事處或其他經外交部授權機構（以下簡稱駐外機構）、行政院設立或指定之機構或委託之民間團體驗證之外國學校最高學歷畢業證書或同等學力證明文件及歷年成績單(中、英文以外之語文，應附中文或英文譯本)。

The highest-level diploma and transcript issued by an overseas academic institution. These documents must be authenticated by a ROC embassy, consulate, representative office, another agency authorized by the Ministry of Foreign Affairs (hereinafter referred to as “Taiwan’s overseas mission”), or verified by institutes established or appointed by, or through a private agency commissioned by, the Executive Yuan. (These documents should be translated into English or Chinese if they are in other languages.):

1. 大陸地區學歷：應依大陸地區學歷採認辦法規定辦理。

Academic credentials from Mainland China: The Regulations Governing the Accreditation of Schools in Mainland China shall apply.

2. 香港或澳門學歷：應依香港澳門學歷檢覈及採認辦法規定辦理。

Academic credentials from Hong Kong or Macao: Academic Credential Verification and Accreditation Methods adopted in Hong Kong and Macao shall apply.

3. 其他地區學歷：

Academic credentials from other areas:

(3-1)海外臺灣學校及大陸地區臺商學校之學歷同我國同級學校學歷。

Academic credentials earned at overseas Taiwan schools or Taiwan schools in Mainland China shall be regarded as the same as those at domestic schools with equivalent levels.

(3-2)上述(1)(2)項以外之國外地區學歷，應依大學辦理國外學歷採認辦法規定辦理。但設校或分校於大陸地區之外國學校學歷，應經大陸地區公證處公證，並經行政院設立或指定之機構或委託之民間團體驗證。

    Academic credentials referred in the preceding 2 Items shall be subject to the Regulations Regarding the Assessment and Recognition of Foreign Academic Credentials for Institutions of Higher Education. However, academic credentials earned from schools or branches established in Mainland China by foreign schools shall require public notarization in Mainland China and be verified and examined by an institute established or appointed by, or through a private agency commissioned by the Executive Yuan.

6. 推薦函二份（包括一份專業科目教師推薦書及一份中文教師推薦書）。

   Two letters of recommendation in English or Chinese, preferably one by a subject    

   course instructor and the other by a Chinese language course instructor.

7. 中文或英文留學計畫書一份。

   A Chinese or English “Statement of Purpose” is required to describe your previous academic history, your strength and weakness, your professional interest, your study plan in Chung Shan Medical University, and your future career planning. Some departments have word restriction. Please check the requirement for your department very carefully.

8. 申請人須具備語文能力及檢附證明文件如下：

(1)本校教學以華語文（中文）為主，如申請系所需具有中文能力，則須具備相當於 華語文能力測驗 TOCFL)A2 級或歐洲共同語文參考架構 (CEFR) A2 級之華語文能力檢定證明。但申請人前段學歷為主修華語文或以華語文授課為主並檢附相關證明者，得免附中文能力證明。

The primary language of instruction in CSMU is Mandarin Chinese. If proficiency in Chinese is required for the program applied for, the applicant must have proof of language proficiency equivalent to TOCFL (Test of Chinese as a Foreign Language) A2 level or CEFR (Common European Framework of Reference for Languages) A2 level. However, applicants with prior educational background in Chinese language or proof of education in Chinese as the primary medium of instruction are exempt from providing Chinese language proficiency certificates.

(2)如申請系所需具有英文能力，應檢附相當於 CEFR（進階級）B1（含）以上之英文 能力證明。但申請人國籍為英語系國家、在英語系國家取得前一學位或前一學位 為全英語授課並檢附相關證明者，得免附英文能力證明。

If proficiency in English is required for the program applied for, the applicant should submit proof equivalent to CEFR B1 level or higher. However, applicants from English-speaking countries, those who obtained their previous degree in an English-speaking country, or have proof of a previous degree conducted entirely in English are exempt from providing English language proficiency certificates.

9. 申請人須具備及提供之語文能力證明文件(依申請系所學制之規定) (影本)。

Applicants must possess and submit language proficiency certificates in accordance with the regulations of the applied program or department. (photocopy)

1. 授課語言規劃標註■及■顏色區塊之系所，申請者應繳交英文能力證明。

For Departments/Institutes marked with the ■ & ■ color blocks in the language notation of the course planning, applicants are required to submit proof of English proficiency.

2. 授課語言規劃標註■及■顏色區塊之系所，申請者應繳交中文能力證明。

For Departments/Institutes marked with the ■ & ■ color blocks in the language notation of the course planning, applicants are required to submit proof of Chinese proficiency.

10. 申請系所規定之其它文件。

   Other things, if any, as required by the Department or Institute that the applicant intends to enter.

11.請銀行出具足夠在臺就學之財力證明(最低額度為美金一萬元)，或由政府、大專校院或民間機構提供全額獎助學金之證明

        Financial proof by Bank that shows financial sustainability for study in Taiwan (the minimum balance should be equivalent to US$10,000), or proof of full scholarship

        provided by a government agency, university, college, or private organization.

※ 若存款證明非申請人本人帳戶，另須檢附資助者親筆聲名之銀行財力證明及保證書(附件三)，說明資助者與申請人之關係，並保證負擔申請人在臺就學所有費用。

If the bank statement is not under the applicant’s name, a bank financial statement of the sponsor and the financial guarantee must be provided (Appendix 3), in which the relationship between the sponsor and the applicant is stated and the support of the applicant’s expense for the duration of the applicant’s studies in Taiwan is guaranteed.
